The Intercollegiate Athletics Department will provide a competitive, culturally diverse and gender-equitable sports program; one which will serve as a window to the University in an environment of integrity and equality that enables student-athletes to achieve excellence in their academic and athletic pursuits. The department strives to recruit academically sound student-athletes who have the desire and motivation to pursue a degree and become responsible citizens. The Intercollegiate Athletics Department personnel and student-athletes shall act with honesty and sportsmanship at all times while representing the honor and dignity of their sport, as well as Urbana University. The Intercollegiate Athletics program is dedicated to a belief that
athletic participation promotes the development of sportsmanship,
good character and a strong work ethic. The overall physical and
mental well-being of the student-athlete is our top priority. University
spirit and community pride will be developed through a successful
athletics program.


Urbana University considers intercollegiate athletics to be an integral part of the college experience and seeks to provide positive and enriching programs that complement and reinforce the educational pursuits, academic success, and athletic excellence of student-athletes. The institution embraces the concept that the student-athlete is first and foremost a student, while at the same time realizing the dividends gained through athletic competition. Integrity, sense of community, teamwork, camaraderie, sportsmanship, self-respect, self-confidence, leadership, and loyalty all enliven and enrich the life of each student who participates in intercollegiate athletics. The overriding goal of the department is to combine excellence in academic preparation with excellence in athletic competition. Therefore, the University is committed to the development and maintenance of an outstanding athletics program. Successful intercollegiate athletic teams are a source of pride among alumni, students, faculty, staff, and other constituencies. Such pride has contributing effects upon the University, including the areas of alumni giving, public relations, student enrollment, and the ability to attract quality faculty and staff. Furthermore, it is evident that a strong intercollegiate athletics program is part of a well-rounded institution dedicated to the service of a broad range of students. The Athletics Department shares the responsibility with others on campus to contribute positively to the physical, social, and educational welfare of each student-athlete. The University is committed to upholding and practicing the established NCAA and affiliated Conference policies and standards governing intercollegiate athletics. Institutional control is maintained through the development and review of athletics policies by the University President,

Urbana University expects student-athletes to maintain normal progress toward graduation and satisfy standards as identified in the institutional catalog. • To promote the student-athlete’s personal development, including physical skill, cooperation, teamwork, sportsmanship, loyalty, and leadership abilities. • To provide for the treatment, recovery, and rehabilitation of ill or injured athletes through the athletic training program, University physician, and allied medical professionals. • To utilize established University resources to provide student-athletes with services for counseling/treatment for personal and psychological concerns well as substance abuse problems. • To employ competent coaches who promote high standards of integrity and ethical behavior as well as sportsmanship. NCAA DIVISION II PHILOSOPHY
Members of Division II believe that a well-conducted intercollegiate athletics program, based on sound educational principles and practices, is a proper part of the educational mission of a university or college and that the educational welfare of the participating student-athlete is of primary Urbana University concern. Members of Division II support the following principles in the belief that these objectives assist in defining the division and the possible differences between it and other divisions of the Association. This statement shall serve as a guide for the preparation of legislation by the division and for planning and implementation of programs by institutions and conferences. A member of Division II: *Believes in promoting the academic success of its student-athletes, measured in part by an institution's student-athletes graduating at least at the same rate as the institution's student body; *Believes that participation in intercollegiate athletics benefits the
educational experience of its student-athletes and the entire campus
*Believes in offering opportunities for intercollegiate athletics
participation consistent with the institution's mission and philosophy;
*Believes in preparing student-athletes to be good citizens, leaders
and contributors in their communities;
*Believes in striving for equitable participation and competitive
excellence, encouraging sportsmanship and ethical conduct, enhancing
diversity and developing positive societal attitudes in all of its athletics
*Believes in scheduling the majority of its athletics competition with
other members of Division II, insofar as regional qualification,
geographical location and traditional or conference scheduling patterns
*Recognizes the need to "balance" the role of the athletics program to
serve both the campus (participants, student body, faculty-staff) and
the general public (community, area, state);
*Believes in offering opportunity for participation in intercollegiate
athletics by awarding athletically related financial aid to its student-
*Believes that institutional control is a fundamental principle that
supports the educational mission of a Division II institution and
assumes presidential involvement and commitment. All funds
supporting athletics should be controlled by the institution. The
emphasis for an athletics department should be to operate within an
institutionally approved budget, and compliance with and self-
enforcement of NCAA regulations is an expectation of membership;
*Believes that all members of Division II should commit themselves to
this philosophy and to the regulations and programs of Division II.


The purpose of intercollegiate athletics is to provide positive
experiences for all student-athletes that participate at Urbana
University. I strive to serve our University and community, with
leadership in athletics through the support of the values that the
University embraces. As a leader on our campus and in our community
my behavior will reflect a person that is respectful of others,
welcoming of all who are connected to our community and I will be a
contributing member of our Urbana University community. I will
support Urbana University and be a positive role model for teammates
and classmates as well as the youth in our community. I will hold
myself and teammates to a higher standard of conduct to ensure a
positive reflection of Urbana University and the Athletic Department. I
understand that public exposure in the competitive arena, and the
media, puts me among one of the most visible groups in the University
community, the City of Urbana, and the area. What I do and the
way in which I do it are often highlighted, placing me in a
public position which requires exemplary behavior. I will
participate in servant leadership experience that will place me
in the community. The manner in which I conduct myself will
be a reflection of the mission and vision of Urbana University I
understand that I am expected to conduct myself in a manner
that is respectful of me, my opponents, the officials, and the
game. I will conduct myself and lead my team through
responsibility, determination, integrity and sportsmanship.

Through these experiences I will become not only a valuable member
of my team and community but also a true champion of the game.
As a student-athlete at Urbana University it is our hope that you
will not only receive an excellent academic experience, but also grow
spiritually through character building relationships integrated with a
solid understanding of what it is to be a true servant leader.

All student-athletes at Urbana University are responsible to be familiar
with and comply with the policies of their team and coach, the Urbana
University Athletic Department, Urbana University (as indicated in the
Urbana University Student Handbook, Academic Department literature
and elsewhere), the NCAA. It is the responsibility of the student-
athlete to make sure he/she has a current copy of all handbooks and
policy books and understands the information in them in. Any student-
athlete who has questions about policies is responsible to ask for
clarification from their Head Coach or the Athletic Director.

It is up to the Head Coach of each sport program to determine if any
and or what team awards are to be given at the conclusion of each
sport season. Head Coaches must adhere to all guidelines under NCAA
Division II Bylaw 16.1 and only non-state budgeted money shall be
used for awards.

Prior to your participation in intercollegiate competition, a team
meeting will be held and you will receive a copy of the “Summary of
NCAA Regulations” for the academic year. You will also have an
opportunity to ask questions to help you understand any information
that may be unclear related to the NCAA regulations. The NCAA
manual is available in the offices of your Head Coach, the Athletics
Director, and the Associate Athletics’ Director. At your team meeting
you will be required to do the following:
*Submit information relating to your eligibility to compete by signing
Part 1 of the Student-Athlete Statement.
*Give your written consent to disclose your education records to an
authorized representative by signing Part 2 of the Student-Athlete
*Give your permission to be tested for drugs in any NCAA
championship certified by the NCAA by signing the NCAA Drug-Testing
Consent Form.
Failure to complete and sign the statement annually will result in your
ineligibility for participation in all intercollegiate competition. The
“Summary of NCAA Regulations for the current academic year” is
reprinted for your review.

Urbana University is committed to operating its athletic programs in a
manner that is consistent with the letter and spirit of the NCAA and
University rules and regulations. To that end, we have instituted a
compliance program that combines the training and education of
coaches and staff to ensure compliance with those rules and prompt
institutional response when rule violations do occur. Our goal is to
educate coaches, staff and students in their responsibilities under the
rules, to develop operating systems within the Athletic Department
and the University that guide staff in their efforts to work within the
rules, and to respond to each rule violation so that we can correct the
situation which led to the violation. We affirm that coaches, staff
members and student-athletes of the Urbana University Athletic
Department have an obligation to report any violations or potential
violation of NCAA or University rules, of which they are aware. The
information will be forwarded to the Compliance Director who will
examine the situation and communicate directly with the Athletic
Director. All violations will be reported to the appropriate athletic
governing body with confidential copies to the Athletic Director,
Faculty Athletic Representative, and President’s Office. No potential
violation is so minor that it will not be examined.
In responding to violations, we will look at such factors as to whether
the violations were intentional, whether any competitive advantage
was gained (e.g., recruiting, competitive, or for the student-athlete
involved), whether a student-athlete’s eligibility was affected, and
whether the violation is recurring. The goal will be to encourage
communication; to seek consistency and accountability; and above all,
to send a strong message that Urbana University is serious about rules
compliance and institutional control.

Each student-athlete is responsible to comply with all team rules as
provided by the head coach and the Athletic Department. These rules
are important because they potentially affect your status as a student-
athlete at Urbana University. Within the intercollegiate athletic
program, all decisions directly related to team performance, i.e.,
playing time, position, amount of athletic grant, traveling squad, are
the province of the head coach, and these decisions are final. A copy
of the team rules that refer to player appearance, training, curfew, and
conduct is given to each team member prior to the beginning of the
season. If you have not received a copy of the team rules, we suggest
that you obtain a copy immediately and keep it with this handbook. All
sports have written rules. You have a right to appeal any decisions
concerning any alleged situation you consider being unjust. If you
have a grievance or complaint concerning personal rights or team
rules, who should you contact?
1. Report the issue to your Athletics Director or Senior Woman
2. If the issue is not resolved request a meeting with the Director of
Athletics, SWA and Faculty Athletics Representative to request a
formal meeting with the Athletics Advisory Committee.

Sportsmanship is a critical lesson to be taught through participation in
Intercollegiate Athletics at Urbana University. It is important that the
Athletic Department develop policies that will deal with inappropriate
sportsmanship behavior. It is also the belief of the Athletic Department
to reinforce positive behavior. The policy will apply to all members of
the athletic family including athletes, coaches, trainers, managers,
graduate assistants, volunteers, and administration. The policy deals
with ejection from competition for unsportsmanlike behavior that is
unacceptable to the game. It would not include such ejections as too
many fouls or false starts. All ejections will fall under the discipline
that is outlined in each sport’s rules book. For several sports the NCAA
has very specific consequences for all ejections, where other sports do
not have any rule that is stand alone in the NCAA rule book. If the
nature of the ejection results from violence or unsportsmanlike
conduct such as fighting or other violent acts, the Athletic Department
will review and address on a case by case basis.
*Conduct standards are established for student-athletes for each sport
by the Head Coach in addition to the Intercollegiate Athletics
*Violations of the standards may results in disciplinary action by the
Head Coach, which may include suspension/dismissal from the team.
*The disciplinary process includes any penalties imposed by the Head
*The student-athlete may appeal the discipline to the Associate
Athletic Director for Compliance. This MUST be done in writing and
filed in person within 10 days of the disciplinary action.
*The Associate Athletic Director for Compliance will review the appeal,
investigate the action and render a decision.
*If the student-athlete does not agree with the decision rendered by
the Associate Athletic Director for Compliance the student-athlete has
10 days from the date of the decision to appeal this decision to the
Director of Athletics. This appeal MUST also be done in writing.
*The Director of Athletics decision is FINAL.
Student-athletes should make every attempt to resolve any
disciplinary action with the Head Coach first. Appeals will not be
considered if a discussion between the student-athlete and the Head
Coach has not taken place.

*Process is reviewed and presented to the coaching staff at initial staff
meeting each August.
*Process is announced and reviewed with the student-athletes at each
team’s opening eligibility meeting.
*Head coaches are required to inform the Director of Athletics and the
Associate Athletic Director for Compliance when a student-athlete is to
be removed from a team roster.
*The Associate Athletic Director for Compliance will deliver the
grievance/disciplinary process to the student-athlete within 48 hours
of removal from the roster.

Urbana University and the NCAA view all forms of hazing as
unacceptable. Hazing has the potential to cause serious physical or
psychological harm and many acts of hazing are illegal. At Urbana
University, hazing is considered incompatible with the philosophy,
values and goals of the Athletic Department.
Urbana University defines hazing (see student handbook) as any activity which is unsafe, abusive, harmful, distressing, embarrassing, humiliating, disrespectful, illegal or otherwise demeaning that is required or implied as being necessary or beneficial as part of the process of joining, affiliating with, being initiated into or determining status in any group, organization, team or department at the university (whether the group has official recognition from the university). Hazing can occur regardless of the willingness to participate. Hazing includes (but is not limited to) 1) Encouraging or requiring any form of alcohol abuse or the use of any other intoxicant or mind altering substance, 2) The administration of any physical or emotional pain or abuse, creation of excessive fatigue, quests, road trips or any other such activities, 3) Wearing of public apparel which is conspicuous and not normally 4) Engaging in public stunts and buffoonery, morally degrading or 5) And any other activities which are not consistent with law or the The university recognizes that hazing may be perpetrated by a group
or organization as a whole, by an individual or a group of individuals
within the group, by former members of a group, current or former
students, or by current or former faculty or staff. In all circumstances,
the university reserves the right to sanction the group, as well as
individual members, whether the hazing was part of the groups’ official

Anyone who has been subjected to hazing or anyone who has
knowledge that hazing is planned should report such information as
soon as possible to the Director of Athletics. The person(s) making a
report of hazing will be asked to reduce their report to a signed written
statement to include all relevant information such as names of those
involved, dates, times, places and a description of what transpired.

Anyone who reports hazing to the university is protected under the
university’s anti-retaliation policy.

Because hazing is such a serious violation of Athletic Department
policy with the potential to endanger the safety and wellbeing of
others, the Athletic Department reserves the right to temporarily
suspend anyone accused of hazing until the Department’s investigation
is complete and/or all legal charges in the matter are fully resolved
when, in the sole discretion of the Athletic Department the accusations
are credible and have been made in good faith. In the case where the
accused is an employee of the university, the director of human
resources will suspend the employee as appropriate to the situation
and this may include suspension of employment with or without pay.
In the case where the accused is a student, the President, Athletic
Director, and Dean of Students will suspend the student as appropriate
to the situation. This may include but is not limited to, suspension of
eligibility to participate in extracurricular activities; suspension from
eligibility to reside in on-campus student housing; or suspension as a
student from the university. Upon receipt of a report of hazing, the
Athletic Director will promptly appoint a special committee on hazing
to investigate the matter. This committee will consist of the Athletic
Director, Associate Athletic Director for Compliance and three Athletic
Department staff members not associated with the case. A detailed
description of the committee, the university’s process for investigating
any report of hazing as well as consequences for those found
responsible for hazing appears in the Urbana University Student


Urbana University prohibits sexual harassment of its students, faculty,
staff and administrators, including student to student and other peer
sexual harassment. The University recognizes the possibility of same
sex harassment. Same sex harassment will be given the same
consideration as harassment by a member of the opposite sex.

As members of the Urbana University Athletic Department, it is
important to be responsible in the use of technologies such as Face
book, My Space and other World Wide Web addresses. As an student-
athlete you are responsible for all content that is published on these
sites. It is advised to use extreme caution when posting any personal
information such as phone numbers, addresses, daily plans and photos
on the web site. Remember www means WORLD WIDE WEB. Anyone
can view these pages including the media and future employers. Any
inappropriate use of these sites can lead to disciplinary actions.

As a student-athlete at Urbana University there will be several
opportunities when local and regional media may request an interview.
In situations where athletes are interviewed, please use discretion
regarding information that is provided. Please avoid responding to any
question that pertains to the following: Internal team conflict, injury
status of student-athletes, negative comments on opponents, false or
misleading information or any comments that will reflect negatively on
your team, staff or Urbana University. All student-athlete interviews
will be arranged by an Athletic Administrator, Head Coach or the
Sports Information Director. As a student-athlete, you will not arrange
private interviews with the media outside of Urbana University.


The coaches determine the awards for each athlete in their sport. The
award amounts are then provided to the Athletic Director for his
approval. The Athletic Office prepares the Athletic Contracts and
Letters of Intent, obtains the required signatures and sends the
athletic award information to the Office of Financial Assistance. The
Office of Financial Assistance then prepares the Athletic Aid Award
Notification Letters and mails those to the athletes together with all
the appropriate documents enclosed.
The coaches determine the renewal award amounts for each athlete in
their sport. The renewal award amounts are then provided to the
Athletic Director for his approval. The Athletic Office prepares the
Athletic Contracts, obtains the required signatures and sends the
renewal award information to the Office of Financial Aid. The Office of
Financial Aid then prepares the Athletic Aid Renewal Notification
Letters and mails those to the athletes. The athletes are provided with
their Athletic Contracts prior to them leaving the campus for the spring
The coaches determine the reduction in aid award amounts for the
affected athlete in their sport. The reduced award amounts are then
provided to the Athletic Director for his approval. The Athletic Office
prepares the Athletic Contracts, obtains the required signatures and
sends the reduction in aid award information to the Office of Financial
Aid by April 15 each year. The Office of Financial Aid then prepares the
Athletic Aid Reduction Notification Letters and mails those to the
athletes. The appeals procedure is outlined in the letter.
The coaches determine if the athletic aid for any of their athletes will
not be renewed. That information is provided to the Athletic Director
for his approval. The Athletic Office sends the non renewal award
information to the Office of Financial Aid by April 15 each year. The
Office of Financial Aid then prepares the Athletic Aid Non Renewal
Notification Letters and mails those to the athletes. The appeals
procedure is outlined in the letter.


All student-athletes must ride to and from Urbana University
sponsored events with their team and coaches. In case of emergency
travel situations student must apply for a waiver of athletics
department travel policy. This release form is available in the athletic
department website. The release statement must be approved and
signed by the Athletic Director prior to the student-athletes release to
travel independent of the team. All Urbana University policies for
students continue to apply to student-athletes when traveling. The
coaching staff is responsible for enforcing all policies and report any
violations to the Director of Athletics should they occur.
Each student-athlete has a set daily per diem for meals. While
traveling meals can be provided as team or meal money can be
dispersed to student-athletes at the coach’s discretion. If meal money
is dispersed to student-athletes, each coach must provided signed
documentation from all athletes and/or coaching staff member that
received actual money. The daily allowance for student-athletes is up
to $15.00 per day.
Lodging is provided for all student-athletes while representing Urbana
University at scheduled events. Each sport coach is responsible for
making team travel reservations at their designated hotel or motel.

Student-athletes participating in the intercollegiate athletics program
are expected to attend classes. A student-athlete who disregards this
responsibility is subject to immediate suspension of eligibility for
athletic participation by the Faculty Athletic Representative or the
Director of Athletics. Appeals of such a suspension can be made to the
Intercollegiate Athletic Committee and/or the Student Grievance
Committee. No class time shall be missed for practice activities
except when a team is traveling to an away-from-home contest
and the practice is in conjunction with the contest.
student- athlete shall provide his/her coach with an accurate and
complete class schedule at the beginning of each term. Any proposed
schedule change must be discussed in advance with the Associate
Athletic Director and Head Coach and appropriate advisor.

All student-athletes must complete an Academic Contract for each
class he or she is taking every semester regardless of whether they
will be missing the class(s) because of scheduled athletic competitions.
The Academic Contract can be found on the athletics website under
“Departmental Forms.” The Academic Contract must be completed and
returned to each student-athlete’s coach with all required signatures
by the beginning of the third week of classes. After each Head Coach
has collected ALL of the ACADEMIC CONTRACTS from their student-
athletes each semester the Academic Contracts are to be turned into
the Associate Athletic Director for Compliance.
After student-athletes register for classes, a hold will be placed on
their ability to add or drop classes. If a student-athlete would need to
add or drop classes during the semester, he or she must meet with the
Associate Athletic Director for Compliance before the hold will be lifted.
Faculty members are asked to assess the progress of all students in
their classes and report to the Registrar by the end of the eighth week
of the term any students needing Academic Intervention. This
information is available to advisors during the ninth week. In addition,
midterm grade and attendance checks will be done for all active
student-athletes throughout each semester.
Each student-athlete is required to pass at least twelve (12) new
semester hours each term and twenty four new semester hours in the
previous two semesters, and earn an overall cumulative institutional
GPA of 2.00 or higher. If this standard is not met, the student-athlete
is ineligible to compete in the subsequent term. In addition, student-
athletes must be enrolled in a minimum of 12 new degree hours at UU
each semester. Concurrent enrollment at another institution does not
fulfill the 12 hour requirement.
A student-athlete must declare a major by the beginning of his or her
third year or fifth semester of enrollment. This requirement applies to
both continuing and transfer student-athletes. From this point, the
credits used to determine academic certification must be degree
credits accepted by UU. A student-athlete must take 75 percent of the
degree requirements during the regular academic year, and he or she
must average 12 credits per semester toward degree completion.
A student-athlete’s participation in required activity with an athletics
purpose is limited to a maximum of four hours per day and 20 hours
per week. (NCAA Bylaw All competition and associated
activities on the day of competition count as three hours regardless of
the actual duration. (NCAA Bylaw One day off is required
each week. (NCAA Bylaw
Outside of the playing season, student-athlete’s participation in
weight-training, conditioning, and individual skill instruction is limited
to eight hours per week. (NCAA Bylaw Two days off are
required each week. (NCAA Bylaw Voluntary activity that is
initiated by the student-athlete without the instruction or supervision
of a coach or athletics staff member is allowed in excess of these
restrictions. (NCAA Bylaw 17.02.14).
A student-athlete must complete his or her eligibility within the first 10
semesters (or 15 quarters) of enrolling in a collegiate institution full-
time. (NCAA Bylaw 14.2.2). A semester is used when the student-
athlete is enrolled full-time on the first day of classes, even if he or
she drops to part-time status. (NCAA Bylaw Even if a
student-athlete is enrolled less than full-time, a semester is used if he
or she participates in intercollegiate athletics. (NCAA Bylaw

All students enrolled at Urbana University will receive free admission to
all UU hosted intercollegiate athletic events when showing a valid I-
All Student-Athletes at UU will be provided up to four (4)
complimentary tickets for each regular season home athletic event in
the sport in which they are a team member. Complimentary tickets
are to be used for immediate family members only (parents,
siblings, children, spouses). Any Student-Athletes found
misusing complimentary tickets for any reason will have this
privilege revoked by the Director of Athletics.
*Head Coaches will provide a completed complimentary admission list
to the UU Athletic game manager by 1:00 pm on the day of the home
event, for events held during the week and by 5:00 pm Friday for all
home weekend events. All names on the complimentary admission list
are to be printed and easily read. No additions or changes to the
list will be allowed after the lists are turned in to the Athletic

The student-athlete is required to pass an annual physical examination
prior to participating in practices and/or competitions. This policy also
applies to any conditioning or training program which is scheduled
and/or supervised by intercollegiate athletics’ personnel outside or
during the sports main competition season. For those sports where
there is a conflict with the beginning of practice and a scheduled
annual physical examination date, returning student-athletes with the
previous year’s UU physical exam on file may be allowed to participate
until his/her team’s scheduled physical exam date. New student-
athletes who have never had a UU team physical may not participate
until the student-athlete has passed the UU team physical.
Athletic training personnel are not permitted to administer treatment
to any student-athlete who does not have a physical examination on
Prior to participation in UU Intercollegiate Athletics, each student-
athlete shall read and sign an Assumption of Risk Statement, a
statement defining the limits of Athletic Insurance Coverage, a
statement granting the release of student-athlete information for
media purposes, and a statement granting the release of medical
information to identified athletic personnel. The insurance policy at UU
is like that carried by most major universities and is commonly
referred to as an “excess policy.” Therefore, if you sustain an injury or
illness while officially participating as a member of an intercollegiate
team, a claim must first be submitted under you or your parents’
group or personal insurance plan. The university athletic insurance
policy will then consider the unpaid balance of any claim not paid by
your policy up to the limits of the program’s coverage. The athletic
administration is responsible for the management of all athletic injury
claims. UU Sports Medicine shall authenticate and file all injury reports
with the Director of Athletics.
The student-athlete is responsible for reporting all injuries to the
sports medicine staff and completing all insurance documentation


The purpose of the Student-Athlete Advisory Committee is to support
the mission of Intercollegiate Athletics at Urbana University by
providing a forum in which student-athletes and administrators,
through the expression of ideas and thoughtful examination of mutual
interests and concerns, enhance the athletics experience at UU for the
student-athletes, the campus community and the community-at-large.
The Student-Athlete Advisory Committee:
*Will support a university environment that fosters a positive athletic
and academic experience for student athletes by promoting a healthy
lifestyle, open communication between student-athletes and the
University community, and increased support among their peers.
*Is committed to serve as the ambassador for Intercollegiate Athletics
to the surrounding communities for the purpose of expressing to its
citizens the potential for, and benefits of, participation in athletics.
*Encourages student-athletes to be goodwill ambassadors of the
University and the Intercollegiate Athletics Program, and take the
initiative in the promotion and development of Blue Knight pride within
the University community.
*Will strive to effectively communicate the mission and goals of
Intercollegiate Athletics to the Student Government at UU in order to
foster a climate of understanding and support.
*Each team shall select one individual to serve as its committee
member, and one alternate. One or both may attend meetings. In
addition, an at-large member may be selected by the Director of
*Student-athletes who have completed at least one year of athletics
eligibility may serve as a committee member.
*The term of office for team representatives begins with the first
meeting of the fall semester and continues until the end of the
academic year.
*The athletic team which elected the student-athlete can revoke
his/her membership by a two-thirds vote.
*If a team is unrepresented by the elected member or his/her
alternate at two or more consecutive meetings, the team may replace
its representatives. Any absence by the elected member and the
alternate shall be reported to the coach of the unrepresented team.
*The Director of Athletics, Associate Athletic Director for
Compliance/SWA (SAAC Advisor), and one Head Coach (selected by
the coaches) shall serve in an ex-officio capacity.
Each team may determine how its representatives are selected.
Officers will be selected by the Committee based on nominations and
elections. The elections shall be determined by majority vote. The
term for officers is one academic year.

presented in writing at a regular board meeting.

The Urbana University Athletic Training Program exists to provide the
opportunity for student-athletes to compete in his or her sport(s) in
the safest possible environment. Student-athletes, regardless of race,
gender, age, or religion, will be treated in an equitable manner. UU
Sports Medicine strives to deliver the highest quality of care for our
student-athletes. This care includes injury prevention, evaluation, first
aid, treatment / rehabilitation, and appropriate medical referrals. The
sports medicine staff strives to:
*Create a comfortable, caring environment.
*Assure that each staff member associated with the athletic training
program is respectful and professional.
*Maintain and operate the athletic training room in an organized,
professional manner.
*Maintain the most current emergency equipment and protocols to
insure safe, efficient care.
*Insure that each athlete is given careful and professional counsel
regarding their injury or illness, and that all available options are

All student-athletes are required to undergo a medical history/
physical examination prior to participation in intercollegiate athletics
for UU. All assigned paperwork must be completed and on file with the
UU sports medicine staff prior to participation. Participation includes
pre-season practice, off-season conditioning, and physical testing. The
UU team physician will give final clearance to participate. A full
physical exam will be conducted for all incoming freshmen and transfer
students. All returning student-athletes are required to fill out an
updated medical history form. If the returning student-athlete has
sustained any injury or illness in the last year, they may be required to
undergo a physical examination prior to being cleared for participation.
Athletic training staff will not be permitted to administer treatment to
student-athletes whose physical examination is not on file.


1. Athletes must sign in for all treatments.
2. Athletes will be seen during posted training room hours unless other
arrangements have been made with athletic training staff.
3. Be on time for treatments. If you are being treated prior to practice
/ game, you must arrive 1 hour before your coach wants you at
practice or game. Anyone arriving last minute for taping or treatment
will first report to practice prior to any treatment being administered.
Coaches will be given daily injury reports on all student-athletes and
their compliance with treatments.
4. All injuries need to be reported to athletic training staff as soon as
5. Once an injury is reported to the athletic training staff, your
attendance in the training room is expected daily until released by the
athletic training staff.
6. If you get out a piece of equipment, return it when you are finished.
7. Training room towels are to remain in the training room.
8. No horse-play or foul language will be allowed.
1. All athletes must shower prior to using the whirlpool. If you are
doing a lower extremity treatment, use must wash your leg beyond
the point to be treated. A full-body treatment requires a shower.
2. No whirlpool treatments are allowed without supervision of the
athletic training staff.
3. No open wounds are allowed in the whirlpool.
4. Bring your own towel if doing a full-body treatment.
5. Inform the staff at least one hour prior to your desire to take a
recovery plunge so that the whirlpool will be ready and available.
6. Turn whirlpool off if you are the last one out.
UU athletic insurance, which provides coverage for your son/daughter
for injuries occurring while participating in an officially scheduled team
practice, game or team travel, is EXCESS or SECONDARY to any
other collectible group insurance benefits. This simply means that any
claim for benefits must first be filed with the group insurance company
providing coverage for your son/daughter through your employer,
spouse’s employer, or your son/daughter’s personal policy. To
facilitate the handling of claims, it is mandatory that athletics have
complete insurance information on file concerning your child, prior to
the first day of participation. Our athletics insurance company will not
process claims unless we provide proof of insurance information. To
document the required information, a Proof of Insurance form
must be filled out completely, returned to the athletic office
and must be accompanied by a copy of your insurance card.
Failure to submit the information to the office will result in
your child being unable to practice or play with the team until it
is turned in.
If your child is covered by an HMO, or your insurance
requires pre-authorization for treatment, it is important that our sports
medicine personnel have access to that information in their file. The
Department of Intercollegiate Athletics requires student-athletes who
are not covered by their parents or their own policy, to purchase
student-health insurance. REMEMBER: the athletic insurance policy
will not cover medical expenses unrelated to official athletics
participation. If your child incurs any medical illness or injury not
occurring during, or as a result of participation in officially scheduled
athletics activity (i.e. physical education class, intramurals, open
recreation, etc.), and your child has no insurance coverage, then the
student-athlete and his/her family will be responsible for all medical
PLEASE NOTE: UU does not have the option of waiving the
requirement of filing with your group insurance. Most employers’ group
insurance allows dependent coverage to be continued to age 23 if the
dependent is a full-time student. DO NOT drop dependent coverage
while your child is participating in intercollegiate athletics.
Claims against your group insurance policy DO NOT increase your
individual insurance premiums. By utilizing both your group insurance
and our athletics insurance, all bills within the limit of the policy should
be paid in full. Within those parameters, you will not be required to
pay your group insurance deductible or any co-insurance amounts. A
maximum of nine (9) months from the onset of injury is allowed when
turning in all bills. It is extremely important to forward all bills and
an explanation of benefits (EOB) from your insurance company
to the UU Office of Student Services Department as soon as

possible. Balance due billing statements is not allowed as we must
have the detailed billing from the provider of service to process the
claim. UU will not be responsible for any medical bills acquired from
non-approved physicians. Student-athletes may seek a private
physician for treatment but will be solely responsible for those bills. In
that instance, notification must be given to the Sports Medicine
Department so open communication between the physician and the
Sports Medicine Department can be maintained.
Should your group or private insurance change, it is your responsibility
to notify the Sports Medicine Department and to provide copies of the
new insurance information.

Urbana University and the Intercollegiate Athletic Department has a
ZERO TOLERANCE POLICY that all student-athletes are required and
expected to follow from the official start date of their sport’s practice
until their team is eliminated from their sports post season
championships. For the sports of volleyball, men’s and women’s
soccer, softball, men’s and women’s golf the policy is in affect as
stated above during the championship segment of your sport and from
the first date of practice until the last practice date or scheduled
competition during the non-championship segment of your sport. Any
student-athlete(s) who violates an Urbana University campus or a UU
housing policy or is charged with violating a State of Ohio law
regarding the use of alcohol and/or illegal drugs during the time period
stated in the above paragraph will be in violation of the UU
Intercollegiate Athletic Department’s ZERO TOLERANCE POLICY.
Violations will be handled as follows:
FIRST OFFENSE- Suspension from the next two scheduled athletic
contests or two weeks which ever occurs first. Student-athlete may
practice with the team during the suspension at the discretion of the
Head Coach.
SECOND OFFENSE- Suspension and dismissal from the team for the
remainder of the season in which the violation occurred. Student-
athlete will not be allowed to practice with the team and athletic
scholarship aid may not be awarded for the next season at the
discretion of the Head coach and the Director of Athletics on a case by
case basis.
THIRD OFFENSE- Suspension, dismissal and elimination from the
Athletic Program at Urbana University for the rest of their eligibility,
athletic scholarship aid will be terminated immediately upon a third
violation of the UU in season ZERO TOLERANCE POLICY.
All coaches are expected to review the ZERO TOLERANCE
POLICY with ALL of the student-athletes and assistant coaches

who are part of your sport program. This policy must be
included in all sports team rules and policies.
Urbana University does not tolerate student-athletes using,
possessing or selling non-therapeutic drugs. As a member institution
of the NCAA, the UU Intercollegiate Athletics Program strongly
supports and enforces this policy. Our policy also does not tolerate the
use, sale, and/or possession of alcoholic beverages during team
activities, team travel, in lodging provided for team use, or in vehicles
provided for team transportation.
Random Drug Testing:
Urbana University may randomly test for drugs. In order to be
eligible for participation, you will be asked to sign a Consent to
Drug Testing form. UU student-athletes shall also adhere to the
UU policy on alcohol and other drugs as delineated in the UU
Student Disciplinary Code. Further, any criminal conviction
and/or University sanction related to the involvement with
non-therapeutic drugs or alcoholic beverages could result in
suspension of one’s scholarship, and/or suspension from the
team, and/or disciplinary action from higher authorities.
UU Policy and Procedures for Alcohol and Other Drugs is
included in this handbook.

The Urbana University Athletics Department is committed to the
physical and mental health and wellbeing of its student-athletes. The
University recognizes that the use of alcohol and certain drugs, legal or
illegal, is not in the best interest of the student-athlete or UU Athletics.
In an effort to combat the use of illicit drugs, UU Athletics has
implemented a comprehensive substance abuse education and testing
program to promote healthy and responsible lifestyle choices for
student-athletes. Student-athletes shall also adhere to University
policy on alcohol and other drugs as delineated in the UU Student
Disciplinary Code. Further, any criminal conviction and/or University
sanction related to the involvement with non therapeutic drugs or
alcohol could result in suspension of one’s scholarship, and/or
suspension from the team, and/or disciplinary action from higher
The purpose of the UU Athletics Substance Abuse and Testing Program
1. To deter the use of illicit drugs and alcohol;
2. To identify banned substance users;
3. To provide educational and rehabilitative services;
4. To counsel student-athletes who do not adhere to the requirements
of the program;
All student-athletes must sign the University consent form in which the
student-athlete consents to random testing substances banned by the
NCAA legislation. By signing the consent, the student-athlete agrees to
submit to any and all tests ordered by UU Athletics in order to detect
unauthorized substance use. This consent allows for the limited release
of test results, as outlined in the following procedures, as a condition
of the student-athletes eligibility. Completion of this document is
mandatory for participation in UU Athletics. All student-athletes
enrolled at UU on the athletic roster are eligible for drug testing. This
includes but is not limited to, red-shirts, academically ineligible and
medically ineligible student-athletes. Student-athletes who have
exhausted their eligibility and
those student-athletes receiving fifth-year athletic financial aid are
subject to drug testing.
UU Athletics utilizes the most current NCAA List of Banned Drug
Classes (NCAA Bylaw for its substance abuse testing
program. This list may be obtained through:
a) The current UU Student-Athlete Handbook/Planner
b) A member of the UU Sports Medicine Department;
c) A member of the UU Athletics Compliance Office; or
d) The NCAA website (www.ncaa.org/sports_sciences/drugtesting).
No substance belonging to the prohibited class (es) may be used,
regardless of whether it is specifically listed as an example. Specimen
manipulation - the attempt to alter the integrity of a specimen - is
considered a positive drug test. Manipulation includes adding any
substance to the urine specimen during or after the act of urination.
All UU student-athletes are subject to drug testing. Selection for the
drug testing will occur randomly throughout a student- athlete’s
association with UU Athletics. The Athletic Director / Sports Medicine
staff will determine the date, time, and site for testing. UU will conduct
random drug testing a minimum of one (1) time every ten (10) weeks,
up to a maximum of once every 24 hours. All student-athletes are
eligible for each test
. Therefore, someone may be tested more than
once per year.
In addition to random drug testing, UU Athletics reserves the right to
screen a student-athlete anytime there is reasonable suspicion that
he/she may be engaged in the use of banned substances. The term
“reasonable suspicion” means that information has been received
by a member of the coaching staff, Sports Medicine Department,
and/or athletics administration, in good faith, from a reliable source or
sources regarding a student-athlete’s use of banned substances.
Furthermore, a student-athlete’s possession or use of a prohibited
substance, arrest or conviction related to the possession of, use or
trafficking of banned substances, and/or abnormal conduct
interpretable as being caused by the use of banned substances, all rise
to the level of reasonable suspicion.
1) Upon selection, student-athletes will be notified prior to testing by a
member of UU Sports Medicine, the student-athlete’s coach, and/or a
member of UU Athletics administration.
a. Personnel will call all available phone numbers in an effort to notify
the student-athlete.
b. Personnel MAY leave a phone message on the student-athlete’s
voice mail.
2) Notification will take place no more than twenty-four (24) hours
before the scheduled test.
3) Upon notification, the student-athlete will be asked to read and sign
an UU Substance Abuse Testing Notification Form, notifying him/her of
the date, time, and site of the testing, as well as any other special
instructions pertinent to the test.
a. The student-athlete will be instructed to report to the test site with
a picture ID.
b. The student-athlete will not be allowed to practice or otherwise
participate in team activities until he/she has reported to the testing
site and supplied a viable sample.
4) It is the responsibility of UU Sports Medicine to notify the Athletic
Director of those individuals that do not report for their test within the
scheduled time periods.
5) If a student-athlete does not report at the scheduled time for
his/her test, he/she will be sanctioned appropriately for his/her action.
A "no show" will be interpreted as a positive test result, and the
student-athlete will enter the UU Substance Abuse Program at the
appropriate level.
UU Sports Medicine or an outside company under contract with the
University will supervise the collection, screening, and analysis of a
urine sample from the student-athlete. At the time of collection, all
samples will be screened for a specific gravity level. If a sample does
not meet the required level of specific gravity, the student will be
required to remain in the drug testing area until an adequate sample
can be obtained, or until released by UU Sports Medicine. If the
sample meets required specific gravity levels according to the methods
available at the time of collection, but does not meet those standards
upon laboratory analysis, the sample may be tested for diuretics.
Collected samples and identification information will remain under
constant supervision of UU Sports Medicine.
Specimen manipulation—the attempt to alter the integrity of a
specimen—is considered a positive drug test. Manipulation includes
adding any substance to the urine specimen during or after the act of
urination, catheterization, urine substitution, or modification of renal
excretion by the use of diuretics, probenecid, bromantan or related-
compounds, and epitestosterone. Any sample may be tested for the
presence of agents used to manipulate the specimen. A positive
finding of any of the previously listed substances used in specimen
manipulation will be considered a positive test and the appropriate
level of sanctions will be imposed.
UU Athletics will also issue a positive test finding to student-athletes
who are convicted or plead guilty to the following:
1. Driving Under the Influence (DUI, DWI)
2. Public Intoxication
3. Drunk and Disorderly
4. Violation of Ohio’s Zero Tolerance Law (625 ILCS 5/11-501.8)
5. Other alcohol and drug offenses, including possession
The consumption of alcohol by a student-athlete is prohibited during
any official intercollegiate team function. An official team function, for
purposes of definition with regard to this policy, is defined as any
activity held at the direction or under the supervision of the team’s
coaching staff.
A student-athlete who consumes alcohol will be accountable for any
alcohol-related incident in which he or she is involved on or off
campus. In such cases, the student-athlete is subject to the UU
Athletics or team disciplinary action dependent upon the incident
having or not having legal implications. If a student is involved in an
alcohol-related incident in which there are no legal implications, the
head coach of that team and the athletic administrator supervising
that program will determine if the circumstances warrant suspension
of the student-athlete from practice and/or competition. A
recommendation may also be made for the student-athlete to attend
counseling. Student-athletes are subject to University sanctions for
conduct associated with the illegal and/or irresponsible possession and
use of alcohol. In the event the Office of Student Affairs imposes
disciplinary sanctions against the student-athlete, the head coach and
the administrator supervising that sport will review the findings and
recommend whether additional sanctions will be imposed and/or
recommend counseling.
Finally, the Athletic Department prohibits the purchase of alcoholic
beverages to be used by any department – affiliated person who is
under the legal drinking age. This applies to PROSPECTIVE student-
athletes while visiting the University and their student hosts.
Regardless of whether a student-athlete or student host has reached
the legal drinking age, purchasing alcohol for consumption by a person
under the legal drinking age (a teammate or, in most cases, a
prospective student-athlete) is a violation of state law.
The use of tobacco products by a student-athlete is prohibited during
practice and competition. A student-athlete who uses tobacco products
during a practice or competition shall be disqualified for the remainder
of that practice or competition (NCAA Bylaw 17.1.9). UU does not
tolerate student-athletes using, possessing, or selling non-therapeutic
drugs. As a member institution of the NCAA, the UU Intercollegiate
Athletics Program strongly supports and enforces this policy. Our
policy covers the use, sale, and/or possession of alcoholic beverages
during team activities, team travel, in lodging provided for team use,
or in vehicles provided for team transportation.
The Athletic Director, Athletic Administration, Head Athletic Trainer,
the student-athlete’s head coach and the team physician may be
informed of test results. Other university employees may be informed
of test results to the extent necessary for the implementation of this
policy and/or for the performance of their duties.
All information and records associated with the UU drug testing policy,
including test results, will remain confidential and, unless otherwise
required by law, may be released only to the following persons:
*Director of Athletics
*Athletics Administration
*Head Athletic Trainer
*Head Coach
*Counseling and Consultation
*Team Physician(s) and appropriate Student Health Center personnel
*Other UU employees, as necessary to implement this policy and/or
for the performance of their duties.
All members of the UU Athletics Department are expected to respect a
student-athlete’s right to privacy. It is essential that anything seen,
heard, read, and/or otherwise obtained remain confidential by all
parties involved. It is illegal for any personnel to gain access to patient
information, through any and all means, unless the information is
needed in order to treat the patient, or because their job would require
such access. All UU Athletics Department personnel are expected to
adhere to the University’s Confidentiality Policy at all times. Violation
of the policy may incur disciplinary action at the discretion of the
Director of Athletics.

A student-athlete may appeal the finding of a positive result, violation
of this policy, or the sanction imposed for either a positive test or
violation of this policy. To appeal the student must provide written
evidence of a material procedural error, evidence that refutes the
positive finding or violation, or evidence that the sanction is
If the student does not appeal within 7 days, the result,
finding or sanction cannot later be reviewed. If the student wishes to
appeal a positive test result, a finding of a policy violation, or a
sanction other than suspension, the Athletic Director will appoint a 3-
member appeals committee to review the evidence and make a
recommendation. The Athletic Director will provide a written decision
to the student. This decision will be final.
UU encourages students to seek assistance for problem drug and
alcohol use. Additional information, literature and confidential
counseling are available to student-athletes through UU Student
Health Services and UU Counseling Center.


NCAA Banned-Drug Classes 2012-13
The NCAA list of banned-drug classes is subject to change bythe NCAA Executive Committee. Contact NCAA education services or www.ncaa.org/health-safety for the current list. The term “related compounds” comprises substances that are included in
the class by their pharmacological action and/or chemical structure. No substance
belonging to the prohibited class may be used, regardless of whether it is
specifically listed as an example.
Many nutritional/dietary supplements contain
NCAA banned substances. In addition, the U.S. Food and Drug Administration (FDA)
do not strictly regulate the supplement industry; therefore purity and safety of nutritional dietary supplements cannot be guaranteed. Impure supplements may lead to a positive NCAA drug test. The use of supplements is at the student-athlete’s own risk. Student-athletes should contact their institution’s team physician or athletic trainer for further information.
Bylaw 31.2.3. Banned Drugs The following is a list of banned drug classes, with
some examples of substances under each class.
No substance belonging to the banned drug class may be used,
regardless of whether it is specifically listed as an example.

(h) Definitions of positive depends on the following:
1for caffeine—if the
concentration in urine exceeds 15 micrograms/ml. 2for testosterone—if the administration of testosterone or use of any other manipulation has the result of increasing the ratio of the total concentration of testosterone to that of epitestosterone in the urine to greater than 6:1, unless there is evidence that this ratio is due to a physiological or pathological condition. 3for marijuana and THC—if the concentration in the urine of THC metabolite exceeds 15 nanograms/ml. Drugs and Procedures Subject to Restrictions. The use of the
following drugs and/or procedures is subject to certain restrictions and may or may
not be permissible, depending on limitations expressed in these guidelines and/or quantities of these substances used: (Revised: 8/15/89) (a) Blood Doping. The practice of blood doping (the
intravenous injection of whole blood, packed red blood cells or blood substitutes) is
prohibited, and any evidence confirming use will be cause for action
consistent with that taken for a positive drug test. (Revised: 8/15/89, 5/4/92) (b)
Local Anesthetics.
The Executive Committee will permit the limited use of local
anesthetics under the following conditions: (1) That procaine, xylocaine, carbocaine or any other local anesthetic may be used, but not cocaine; (Revised: 12/9/91, 5/6/93) (2) That only local or topical injections can be used (i.e., intravenous injections are not permitted); and (3) That use is medically justified only when permitting the athlete to continue the competition without potential risk to his or her
health. (c) Manipulation of Urine Samples. The Executive Committee bans the
use of substances and methods that alter the integrity and/or validity of urine samples provided during NCAA drug testing. Examples of banned methods are catheterization, urine substitution and/or tampering or modification of renal excretion by the use of diuretics, probenecid, bromantan or related compounds, and epitestosterone administration. (Revised: 8/15/89, 6/17/92, 7/22/97) (d) Beta 2 Agonists. The use of beta 2 agonists is permitted by inhalation
only. (Adopted: 8/13/93) (e)Additional Analysis. Drug screening for select
nonbanned substances may
be conducted for nonpunitive purposes. (Revised: 8/15/89)

1. Only those persons authorized by the Head Athletic Trainer will be
allowed in the collection station.
2. The Head Athletic Trainer may release a sick or injured student-
athlete from the collection station or may release a student-athlete to
return to competition or to meet academic obligations only after
appropriate arrangements for testing of the student-athlete have been
made and documented on the Student-Athlete Notification Form.
3. Upon entering the collection station, the student-athlete will show
photo identification (Urbana University ID-Card or Drivers License) to
the Head Athletic Trainer. The student-athlete will record time of
arrival and print name on the Student-Athlete Roster Form.
4. When ready to provide urine, a crewmember will escort the student-
athlete to collection area.
5. The student-athlete will select a sealed beaker from a supply of

observation in order to assure the integrity of the specimen.
7. Once a specimen (at least 40-ml) is provided, the student-athlete is
responsible for keeping the collection beaker closed and controlled.
8. Water given to student-athletes who have difficulty voiding must be
from sealed containers (certified by the crew chief) that are opened
and consumed in the station. These items must be caffeine-and
alcohol-free and free of any other banned substances.
9. If the specimen is incomplete, the partial specimen (less than
40mL) will be discarded and the sample collection continued until the
donor can provide a single urine sample of acceptable volume.
10. If the student-athlete is unable to void and the student-athlete
must leave the collection station for a reason approved by the crew
chief, upon return to the collection station, the student-athlete will
begin the entire collection procedure again.
11. The laboratory will make final determination of specimen
12. If the laboratory determines that a student-athlete’s specimen is
inadequate for analysis another specimen may be collected.
13. If a student-athlete is suspected of manipulating specimens (e.g.,
via dilution), UU will have the authority to perform additional tests on
the student-athlete, not to exceed two consecutive negative tests.
14. The crewmember that monitored the furnishing of the specimen by
observation will sign the Student-Athlete Signature Form.
15. The student-athlete will place the cap on each vial; the
crewmember will then seal each vial in the required manner under the
observation of the student-athlete and the head athletic trainer.
16. The laboratory’s copy of the Student-Athlete Signature Form shall
not contain the name of the student-athlete.
17. All sealed specimens will be secured in a shipping case. The Head
Athletic Trainer will put the laboratory copy of the Student-Athlete
Signature Form in the case, and prepare the case for forwarding.
18. The student-athlete and witness will sign the Student-Athlete
Signature Form, certifying that the procedures were followed as
described in the protocol. Any deviation from the procedures must be
described and recorded on the Student-Athlete Signature Form at that
time. If deviations are alleged, the student-athlete will be required to
provide another specimen.
19. The crewmember will sign the Student-Athlete Signature Form,
give the student-athlete or a designee a copy and secure all remaining
copies. The compiled Student-Athlete Signature Forms constitute the
“Master Code” for that drug testing.
20. After the collection has been completed, the specimens will be
forwarded to the laboratory and all copies of all forms forwarded to the
designated laboratory.
21. The specimens become the property of the designated laboratory.
22. Failure to sign the Student-Athlete Notification Form or the
Student-Athlete Signature Form, arrive at the collection station at the
designated time without justification, or not provide a urine specimen
according to protocol is cause for the same action(s) as evidence of
use of a banned substance. The Head Athletic Trainer will inform the
student-athlete of these implications (in the presence of witnesses)
and record such on the Student-Athlete Notification Form. If the
student-athlete is not available, the Head Athletic Trainer will notify
the Director of Athletics. The student-athlete will be considered to
have withdrawn consent and will be ineligible on that basis.
1. The Head Athletic Trainer will package all specimen samples in a
shipping container, securely seal the container, affix the shipping label,
and arrange for pickup via the laboratory’s designated commercial
2. The securely sealed container will be placed in the custody of the
Program Assistant of Intercollegiate Athletics until the courier arrives
for pick up.
3. A laboratory representative will receive the shipping case(s) and
deliver them to the carrier.
4. A laboratory employee will record that the shipping cases(s) have
been received from the carrier.
5. The laboratory will record whether the numbered seal on each vial
arrived intact.
6. If a specimen arrives at the laboratory with security seals not intact,
the student-athlete may need to provide another specimen.

1. Laboratory will use a portion of the specimen for its initial analysis.
2. Analysis will consist of sample preparation, instrument analysis and
data interpretation.
3. The laboratory director or designated certifying scientist will review
all results showing a banned substance and/or metabolite(s) in

Athletics of the results by each respective code number. Subsequently,
the laboratory will mail the corresponding written report to the
Director of Athletics.
5. Upon receipt of the results, the Director of Athletics will break the
number code to identify any individuals with positive findings.
6. For student-athletes who have a positive finding, the laboratory will
contact the Director of Athletics or his/her designate by telephone as
soon as possible. The telephone contact will be followed by an
“overnight/signature required” letter (marked “confidential”) to the
Director of Athletics. The Director of Athletics shall notify the student-
athlete of the finding.
7. The laboratory will maintain all negative specimen samples for
seven (7) days; all positive specimen samples are maintained for 12
8. Upon notification of a positive test, during the telephone
conversation, the laboratory representative will advise the Director of
Athletics that the remaining specimen will be available for transferring
to a separate facility for additional testing. The student-athlete will be
provided with a list of approved accredited testing laboratories for
alternative testing.
9. Copies of the report from the laboratory that contain results from
the positive specimen will be forwarded to the Director of Athletics
before the appeal call.
10. Technical experts may serve as consultants to the Athletics
Program in connection with such appeals.
11. The Head Athletic Trainer may serve as a consultant to the
committee in appeal phone calls involving matters of collection
